Decreased Level (thrombocytopenia)
Symptoms: bleeding, bruising, petechia on legs
Common causes: medications, myelodysplastic syndrome
Less common causes:sepsis, leukemia, bone marrow diseases, autoimmune diseases, hereditary diseases
Increased Level (thrombocytosis)
Symptoms:usually none.
Common causes:iron deficiency, inflammation, history of splenectomy
Less common causes: bone marrow diseases, leukemia, autoimmune diseases
1. Take Vital Signs

2. Evaluate Symptoms and Signs
Fainting, dizziness or lightheadedness when standing up
GI: Bleeding gums, rectal bleeding
GU: Heavy menstrual bleeding, blood in urine
Skin: any new skin condition, i.e., bleeding, bruising, petechia, rash
3. Take Action using SBAR Report:
Presence of other, significant symptoms or signs of illness
Refer to appropriate Situation-Specific Evaluation for the identified symptoms and signs.
Immediately notify the medical staff & resident representative
Platelet Count less than 20, OR
Platelet Count less than 50 with associated bleeding
Notify medical staff on the next business day
Platelet count < 50 or > 1000
Notify medical staff at the next regular rounds

4. Implement Management Plan
Place on 24-hour report
Obtain lab results (if ordered), and notify medical as needed of significantly abnormal values in lab tests (refer to appropriate Situation)
Update care plan regarding bleeding risk
Review status and plan of care with resident representative
